a) Coordination and liaison between the officials at the provincial departments of Health and Public Works or the implementing agents was insufficient to ensure that newly constructed hospitals, clinics and health centres, or sections thereof, were used optimally at the time of commissioning. In the Eastern Cape, Free State, KwaZulu-Natal, North West and Western Cape provinces, insufficient synchronisation between the times of commissioning, the appointment of staff and the availability of certain equipment and services led to the hospitals and clinics not being used optimally.
