(1) With regard to the recent reports of essential drugs at state hospitals and clinics experiencing stockouts, for each province, (a) which drugs were in short supply or still are in short supply, (b) which conditions do these drugs treat and (c) what was the (i) extent and (ii) reason for each shortage; (2) whether any of the stockouts were due to (a) incorrect quantities ordered, (b) inaccurate forecasting, (c) stock management issues, (d) contracts not being submitted timeously and/or (e) any other reason not related to shortages of pharmaceutical ingredients; if so, what are the relevant details with regard to each province; (3) whether he has implemented any measures to prevent further stockouts; if not, why not; if so, what are the relevant details?
